Please read the following guidelines before applying for financial aid/assistance:

• The applicant must have a Manitoba Health Card & valid Driver's License.

• If you are applying for someone on their behalf, you understand that you cannot accept funds/donations for someone else. The recipient must be aware you are filling this out on their behalf.

• If the application is approved, all donations/funds MUST be signed for by the individual receiving any support from Compassion Connections prior to delivery.

• Not all applicants may qualify. 

• One application per household.
